FROM THE NURSE
We are looking forward to having most of you back on campus, hopefully the near future. As we prepare for the return of our students to school, we kindly ask your parents to fill out prescription medication administration forms for those students who require medications during school hours. Submit both the form and the medication to the health office. As well, food allergy accommodation forms for those students with any allergies. These forms can be found on homepage under the Health office page section. These require a physician signature for modified meals at school.
Frontline is a COVID screening tool that will be required daily prior to students attending campus. There is an app that you can download on your mobile device called, “Frontline Health Portal”. Or you can access it online. Instructions available in Spanish and English, on FHS web page tab Health Office Forms.
Stay well.
