Birthday Marquee Application

For a $25 donation (check or cash) to the Carl Sundahl PTA, your child can have their name displayed on our school marquee to celebrate their special day.

All requests/orders must be submitted and paid for at least 2 weeks before the requested date with the following information listed below.
