     Attention New Hires

    The Employee Benefits Department is no longer creating benefit packets. However, we have created new hire benefit folders on this website which contains all the documents necessary for you to enroll onto our health benefits. View left-hand column for appropriate folder.  Keep in mind that you will have 30 days from your hire date (date you start your new job; contact Human Resources to assist) to submit enrollment forms to the Benefits Department.  Otherwise, you will have to wait for Open Enrollment in May, 2018 to enroll.

    The information required on the medical, dental, vision and life insurance forms will be your name, address, birth date, social security number and your dependents' information (name and birth date) only if you want to enroll them.  If you are enrolling onto Kaiser or Western Health Advantage, you will need to indicate at the top of the enrollment form which plan you are selecting (i.e., Kaiser $20, Kaiser High Deductible, Western Health Advantage $20, Western Health Advantage Hospital (420) or Western Health Advantage High Deductible).

    Once you have reviewed the 2017-2018 New Hire document that shows your monthly cost per paycheck and completed your enrollment forms, send all four forms (medical or waiver form, dental, vision and life) to the Benefits Department.  Faxing at 916-294-9022, mailing or dropping-off forms at the Education Services Center will be the most secure route since you are listing personal information.  If you need help or have questions, you can contact me at asimenta@fcusd.org or Chantal at cmurphy@fcusd.org.

    Qualifying Events Reminder:  Outside of the District's annual Open Enrollment period, you cannot add, drop, or change your election until the next Open Enrollment period unless you experience a "Qualified Status Change" or HIPAA Special Enrollment event as defined by the IRS.  The following are examples of "Qualified Status Changes"; other changes may be permissible:


    • Marriage, domestic partnership, divorce or legal separation
    • Birth or adoption
    • Death of a dependent
    • Change in eligibility for you and/or your dependents under an employer or state sponsored plan including but not limited to:
      1. - Termination of employment for you and/or your dependents
      2. -  Becoming newly eligible for your spouse's employer's plan
      3. -  Your dependent child attaining age 26
      4. -  Dependent child becoming disabled


    You must notify the District's Benefits Office of any of the above events within 30 days of the event.  Failure to do so will result in a forfeiture of COBRA rights and responsibility for any premium contributions taken for dependents who are no longer eligible or forfeit your right to enroll in the District's plans until the next open enrollment period.
