Complete the Preschool Intake Packet and submit copies of all of the following required documents:
Health history and parental consent forms
Copy of your child's birth certificate (or hospital birth announcement, passport or baptism record)
Copy of your child's immunization records
Proof of residency in our school district (such as your driver's license, voter registration information, rental or mortgage statement, pay stub or household utility bill)
Once all the above mentioned documents have been received by us, a specialist from the Early Childhood Assessment Team (ECAT) will contact you to discuss the process for Special Education assessment and services.
