Preschool Assessments - Folsom Cordova Unified School District

If your preschool-aged child needs to be assessed for Special Education services, please follow these steps.

1. Complete Inquiry Form

As a first step, please complete the Preschool Assessment Inquiry Form to notify ECAT of your concerns regarding your child's development.

Once your form has been submitted, a member from ECAT will review your information and contact you regarding next steps.

After your Preschool Assessment Inquiry Form has been reviewed, submit copies of all of the following required documents:

Copy of your child's birth certificate (or hospital birth announcement, passport or baptism record)
Copy of your child's immunization records
Proof of residency in our school district (such as voter registration information, rental or mortgage statement, pay stub or most current household utility bill)
