WORK-RELATED INJURY/ILLNESS REPORTING INSTRUCTIONS:

       If an Emergency (life threatening injury/illness):

  • Call 911 or have someone take you to the nearest hospital.
  • As soon as possible report the injury/illness to your supervisor, or Contact FCUSD's Workers' Compensation Analyst to report the injury/illness.
  • You or your supervisor should call the Early Intervention Nurse (EIN) at 1-877-742-3467.
  • Complete the FCUSD Accident/Incident form (see Administrative Assistant on site or contact Tanya Larson)

For Non-Emergency injuries/illness:

  • Employees should immediately report work injuries/illness to their supervisor.
  • After reporting to your supervisor, call the EIN Hotline 1-877-742-3467.
  • Complete the FCUSD Accident/Incident form (see Administrative Assistant on site or contact Tanya Larson)

After-Hours injuries/illness:

  • Report work-related injuries/illness to your supervisor as soon as possible.
  • If medical attention cannot wait until the next business day, go to the nearest urgent care facility.
  • Call EIN Hotline 1-877-742-3467. You may leave a message with your contact information and EIN will return your call on the following business day.
  • Complete the FCUSD Accident/Incident form (see Administrative Assistant on site or contact Tanya Larson)
What to Expect:

When you call the EIN Hotline, you will speak with a qualified Registered Nurse who will ask you questions in order to gather the information needed to complete a medical triage and will work with you to determine whether formal medical care is needed. If formal medical care is needed, the Nurse will arrange your initial medical appointment and will provide you with the date/time and location of the medical appointment.

After your initial medical appointment, the physician will assess your injury/illness, provide a treatment plan, address your work status, and if needed, schedule a follow-up appointment.

After each medical appointment, you will be given a work status report from the physician. The work status report should be provided to your supervisor or Tanya Larson on the same day, whenever possible, or at the beginning of the next workday.
 
Your supervisor and the District will review all medical work restrictions and work diligently to accommodate them.
 
The District contracts with Schools Insurance Authority (SIA), a third-party administrator to manage the workers' compensation claims on behalf of the District. SIA will mail you informational materials regarding workers’ compensation, your claim number, and the claims examiner’s contact information.

Please note: In the event of non-emergencies, seeking medical treatment before reporting the injury to your supervisor and SIA’s Early Intervention Nurse may be considered self-procured medical treatment and the District may not be responsible for payment.
