  • Thank you for interest in volunteering at Folsom Middle School! We encourage you to be active members of our school community and appreciate the time you give to our school.  

    Volunteer Guidelines/Procedures have been put in place by the district to ensure student safety.

    You will need the following to volunteer at FMS:

    • Volunteer Application - signed and approved by Principal
    • Negative TB Test - dated within the last four years.
    • Fingerprints - to be completed after Step 1 + 3

    PREVIOUS VOLUNTEER? If you have completed your volunteer paperwork at another FCUSD site, please contact your previous school and have them send over your volunteer paperwork to FMS for review.

    Category 1 Volunteers: Category 1 clearance is required if you will be working with students but will be supervised at all times by a staff member.  
    • Fill out Category 1 Volunteer Application
    • Attend a Volunteer Orientation OR acknowledge that you have reviewed the Volunteer Handbook; Complete the Volunteer Agreement Form.
    • View the Medical Provider Network information and return the form acknowledging you received it; once on file it remains on file.
    • Provide a copy of your Driver's License or other formal ID to be placed on file at the Office.
    • Obtain a TB Clearance: Provide a current TB test result that reads 'negative' (meaning you do not have TB).  You may submit a printed copy of your test results or immunization records online. If you need to obtain a current TB test, you may go to your health care provider or at one of the district's monthly TB Clinics.  Please note, TB tests take 3-4 days to perform and two visits to your doctor (one to prick you, one to check for a reaction, several days later). TB tests are good for 4 years and remain on file until they expire.
    • File an Emergency Card at the FMS Office.
    • Upon Principal's approval, Category 1 Clearance is granted and the volunteer will be notified of clearance approval 
     
    Category 2 Volunteer: Category 2 clearance is required if you will be alone with students at any time. 

    If you already completed Category 2 clearance with another school, please contact that school directly and have your paperwork sent to FMS. If you have been fingerprinted for another FCUSD site, your clearance may be on file and you may not have to get fingerprinted a second time. Please speak with our parent coordinator to verify if we have your information on our list of cleared volunteers.

    If you have completed Category 2 clearance with FMS before, check with our office to see if any information needs to be updated.

    Much of the application will carry over, however, you will still need to fill out some of the site-specific forms listed below.

    1.  Complete the Category 2 level paperwork and return it the Parent Coordinator at the FMS Front Office:

    • Complete Category 2 Volunteer Application Form. If you have previously completed a form, visit the FMS office to update for the current year.

    2. When FMS completes the paperwork, they will provide you with the signed forms that you will take to the District Office. 

    • Set up an appointment online to get your fingerprints at the District Office. (DO NOT GO TO THE D.O. UNLESS FMS STAFF HAS SENT YOU WITH THE PROPER PAPERWORK)
    • The fingerprinting costs $25, plus an additional $17 if you have lived outside of California in the last 5 years. 
    • Once fingerprinted, by the District (sorry other clearances do not count), your Category 2 status is good for as long as you remain in good standing with the DOJ.
    • Have a photo taken so that you can receive a Volunteer ID Badge. This will be done at the district office.
    • When your fingerprints have cleared (usually within 2 weeks) our district office will send FMS your approved picture badge.
    • Your volunteer badge must be kept in the school office when not in use.  When you sign-in to volunteer in the classroom or go on a field trip, simply pick up your badge and drop it off when you sign out.
