Income Verification Forms needed!
At Cordova Villa Elementary, every student in our school receives free meals. However, we're asking that you complete an important “alternative income eligibility” form.
This is not an application for free or reduced meals. This ensures your school receives the most amount of funding available from the state, which is used to increase services for your school, including intervention teachers, academic coaches, special programs, technology, field trips and more!
We need your help: Without everyone’s participation, your school may not receive all of the funding it is entitled to from the state - meaning fewer services for our students.
Confidential: We will never share your information with anyone. Our system is confidential, and you can return your form in a sealed envelope if requested. You also can fill out the form online in the privacy of your home.
Complete the form: Click here to complete the form online, or call your school. You may also call our Categorical Programs department at (916) 294-9000.
