The following information must be brought to each
bi-monthly meeting. It should be ready to turn in at the
beginning of your meeting with your Educational Advisor.
This information is required for verification
of attendance.
1. Educational Learning Form (ELF): The ELF lists the
general goals in each subject and includes specific
concepts and content areas that will be covered during
the learning period. It contains suggested assignments
for meeting State Standards. It should be used as a
guide in your daily lesson planning. The ELF must be
returned and attached to the work samples. It is signed
by the student, parent, and educational consultant
agreeing to the time and date of the next meeting. Click

2. Daily Log: Entries must be made in the Daily Log
for each day a student is to receive credit for school
attendance. It must be filled out daily noting the topic
or standard worked on along with page numbers for each
subject. A minimum number of hours of quality
educational time are expected each week:
Kindergarten:17.5 hours minimum per week(3.5
hours per day)
Grades 1-3: 23.75 hours minimum per week(4.75 hours
perday)
Grades 4-8: 25 hours minimum per week (5 hours per
day)
Under PE, list specific activities and time spent. A
minimum of 20 minutes per day or 100 minutes per week is
expected for grades 1 through 8. A minimum of 15 minutes
per day for kindergarten is expected.
Parents are also required to verify that their child
did the required number of hours by initialing each
daily entry. The Daily Log must also reflect activities
that would require the number of hours being claimed for

3. Work Samples: Bring all work or evidence of
activities to your bi-monthly meeting. Each piece of
work must have: (1.) student's name (first & last), (2.)
date, (3.) the subject,(4.) the specific assignment, and
(5.) the parent’s initial with evidence indicating that
they have reviewed the work.
Your Educational Consultant will need to look at all of
the work and will choose two samples per subject for
each two week period to be included in the student's
work packet. If your consultant doesn't see evidence of
the work completed, attendance credit cannot be given.
